Objective: To describe the organization of care for type 2 Diabetes Mellitus within the Health Care Network in Manaus, with emphasis on the integration among primary, secondary, and tertiary levels. Method: Medical students conducted observational visits between March and July 2025 to basic health units, specialized outpatient clinics, and municipal and state management bodies, using a structured guide. Result: Articulated actions were identified across the three levels of care, highlighting the role of primary care in diagnosis, follow-up, and referral. Functional integration with specialized services and intersectoral bodies, such as the Food Security Council, was observed. Conclusion: The experience revealed an organized flow of care for type 2 Diabetes Mellitus, with interconnection between the levels of care and a strengthened role of primary care as the coordinator of services, promoting continuous monitoring and health promotion practices.
